Advanced Care Directive

Appointments are available every Monday at Council's offices in Goolwa

An Advance Care Directive is a legal document that gives those who care for the frail-aged a clear understanding of their needs and wishes, should they lose the ability to communicate for themselves. Once completed, this document can be lodged with family, health care providers, GP's and the local hospital.

A volunteer from the Southern Fleurieu Health Service will be in attendance, at the Alexandrina Council offices in Goolwa, every Monday between 10.00am and 1.00pm, trialling the Advance Care Directive (ACD) Assist Program.

The Advance Care Directive Assist Program is conducted by a group of volunteers and provides support to individuals needing help to complete these forms. Witnessing of completed ACD forms is undertaken through the Fleurieu Justice Association to ensure compliance with relevant legal requirements. The documentation will be available for purchase from Alexandrina Council.

Advance Care Directive (ACD) kits are available for purchase prior or at the time of appointment, from the Goolwa & Strathalbyn Office.

  • Kits are $5.00 each and include 2 application forms
  • Additional forms are available for purchase at $1.00 each

Council thanks Dr Rob Thornton, Gerontologist, for his foresight in trialling this project within the Alexandrina District.
